We've been running our DirectAccess implementation without prolems for a couple of years now. Recently however we have noticed that several ISPs are issuing routers for home broadband or mobile broadband connections which issue client machines with both a NAT'd IPv4 Address e.g. 192.168.1.100 and a local IPv6 address.
In this instance the client is no longer able to connect to the directaccess server. The only solution is to untick the IPv6 option from the relevant NIC, after a reboot it starts working again.

Does anyone know of a better solution that unticking IPv6? Or why this happens in the first place?
